This website is operated for Delight Specialist Diagnostic Clinics & Research Laboratories. The public forms are designed for general enquiries and appointment requests only. They are not a patient portal and must not be used to submit medical records, detailed clinical histories, test results, passwords or financial credentials.
Information we collect
When you submit a form, the site records the name, contact details, broad service selection, preferred date if supplied, preferred contact method, and general message that you choose to provide. A short-lived, non-reversible technical identifier may be used to limit automated spam; the raw internet address is not added to the enquiry record by this plugin.
How the information is used
The information is used to respond to your request, confirm availability, provide preparation information, manage the clinic inbox and maintain an appropriate administrative record of the communication.
Sharing and email delivery
Form submissions are stored as private entries in the WordPress administration area and may also be sent to the clinic email address through the website host’s mail system. They should be accessed only by authorised administrators and clinic personnel who need the information to respond. The plugin does not sell enquiry information.
Retention and security
Website administrators should retain enquiries only for as long as reasonably needed and should delete them securely when no longer required, subject to any applicable clinical, legal or operational record obligations. WordPress, plugins, hosting and administrator accounts must be kept updated and protected with strong access controls.

You may choose to contact the clinic by telephone rather than use the form. You can also ask the clinic to correct or review the contact information you supplied, subject to applicable record obligations.
